Does your employer cover GLP-1s for weight loss? What surveys show

Probably not. In KFF’s 2025 survey of employers, about one in five large firms offering health benefits said they cover GLP-1 drugs such as Wegovy for weight loss [1]. Whether yours does is a choice your employer makes, so the answer sits in your plan documents, not in the insurer’s name on your card.

What the KFF employer survey found

KFF’s Employer Health Benefits Survey is an annual survey of employers. Its GLP-1 question was asked only of firms with 200 or more workers [11], so there is no figure here for small businesses.

Most of the rest said no. Among large firms, 57% said they do not cover these drugs for weight loss, and 24% did not know whether their largest plan does [2].

Coverage often comes with conditions. Of the firms that cover the drugs for weight loss, 34% require enrollees to see a dietitian, case manager or therapist, or join a lifestyle program [6].

Why employers hesitate

Among firms that cover them, use was higher than expected for 44% of firms with 1,000 to 4,999 workers and 59% of the largest firms [7]. A “significant” hit to drug spending was reported by 43% and 66% of those same groups [8]. Among large firms not covering the drugs, only 1% said they were very likely to start within a year, and 67% said they were not likely [9]. KFF’s conversations with employers found some had stopped covering the drugs for weight loss, and a few had tightened diabetes coverage too [10].

Insurers see a similar share. Cigna said about 20% of its employer clients had added weight-management GLP-1 coverage by the start of 2025 [13].

Your employer decides, not your insurer

Most people with job-based coverage are in self-funded plans: 67% of covered workers, and 80% at larger firms [12]. In those plans the employer pays the claims, and the insurer mainly runs the plan. Insurers’ own pages say the choice is the employer’s:

  • Aetna tells employers they can include or exclude GLP-1 coverage for weight management [14].
  • Premera says weight-management GLP-1s are covered only when the employer group has chosen that benefit [15].
  • Blue Cross and Blue Shield of Illinois says GLP-1 weight management drugs are not a standard benefit [16].
  • Blue Shield of California says that as of January 1, 2026, weight loss drug coverage varies across its large group plans [17].
  • Independence Blue Cross says members whose employer includes a weight loss benefit still have GLP-1 coverage [18].

That is why two people with the same insurer can get different answers.

Covered, but with a higher bar

Some employer options cover only people at higher risk. Premera’s Access Select option sets a BMI threshold of 35, or 27 to 34 with two related health conditions [19]. Cigna’s EncircleRx program requires a BMI of 32 or more, or 27 or more with two weight-related health issues [20]. Express Scripts notes that some patients must join a lifestyle program before getting the drug, a rule chosen by the employer or health plan [21]. How to check your own plan

  1. Search your plan’s drug list. A formulary is the list of drugs your plan covers [23]. Aetna, for example, tells members to check plan documents or the member website [22]. Look for Wegovy, Zepbound and Foundayo, and note any prior authorization flag.
  2. Ask HR or the benefits office directly whether the plan covers GLP-1s for weight loss, not just for diabetes. They made the decision and can say whether it changes at renewal.
