GLP-1 constipation: how common it is and what helps

Constipation is a common side effect of GLP-1 medicines, and it behaves a little differently from nausea: on some labels it does not rise with the dose. On Zepbound, for example, it was reported by 17% of people on 5 mg and 11% on 15 mg in the weight trials [2]. Most of the time it is a nuisance that diet, fluids and activity can help with, but the labels also describe rare cases of blocked bowels.
How often it happened in the trials
The table compares each medicine with placebo, a dummy shot or pill taken by a comparison group. The Zepbound row counts constipation and hard stools together [3]. Column order comes from each label’s table header.

In a separate heart outcomes trial of Mounjaro in people with type 2 diabetes and heart disease, constipation was reported by 13% [14]. The Wegovy tablet trial had side effects similar in type and frequency to the injection table [15].

How long it lasts
The labels do not say. When the Zepbound label describes symptoms that eased after the dose-raising period, it names nausea, vomiting and diarrhea, not constipation [16]. The Mounjaro and Foundayo labels use the same short list [29] [30]. So there is no label basis for expecting constipation to fade on its own after a few weeks. If it continues, bring it up at your next check-in rather than waiting it out.
What helps, according to NIH
MedlinePlus and the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) give general advice for constipation from any cause. None of it is written for GLP-1 medicines, and eating less overall can make some of it harder to follow.
- Move. MedlinePlus suggests walking, swimming or other activity at least 3 or 4 times a week [18].
- Drink. It suggests 8 to 10 cups of liquid a day, mostly water [19].
- Add fiber slowly. Adding it too fast can cause bloating and gas [20]. NIDDK says adults generally need 22 to 34 grams a day, depending on age and sex [21].
- Do not hold it in when you feel the urge to go [22].
- Laxatives, short term, with advice. NIDDK says a health professional may recommend a laxative for a short time [23]. MedlinePlus says not to take one if you have severe stomach pain, nausea or vomiting, and not for more than 1 week without checking with your provider [24].
- Ask about your other medicines. Some medicines and supplements cause constipation, and NIDDK says to talk with your doctor if you suspect one [25].
One source does write about GLP-1 treatment. A 2025 joint advisory from four obesity and nutrition groups (the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association and The Obesity Society) says constipation is common with weight loss and should be managed proactively, starting with enough fluids and fiber from food, though more is often needed [31]. If diet is not enough, it lists daily magnesium, fiber supplements or capsules and polyethylene glycol 3350, and says stool softeners can help avoid straining [32]. None of the GLP-1 labels recommends these products, so ask your prescriber or pharmacist which, if any, fits you.
When to call a doctor
MedlinePlus says to contact a provider if you have not had a bowel movement in 3 days, feel bloated or have stomach pain, are nauseated or vomiting, or see blood in your stool [26].
The labels add warnings specific to these medicines:
- Blocked bowel. After Zepbound went on sale, reports came in of ileus (the gut stops moving), intestinal obstruction and severe constipation with fecal impaction. Because these reports are voluntary, the label says their frequency cannot be reliably estimated [17].
- Severe stomach problems. Tell your provider about stomach problems that are severe or will not go away [27].
- Pancreatitis. Stop the medicine and call right away for severe stomach pain that will not go away, with or without vomiting, sometimes spreading to the back [28].
