Ozempic face: what it is and what the research says

“Ozempic face” is a nickname, not a diagnosis. It describes a face that looks hollower, looser or older after fast weight loss on a GLP-1 medicine, because fat and volume in the face shrink along with the rest of the body [1] [2]. It is not listed as a side effect on the GLP-1 labels we checked, and none of them gives a rate for it.

What it looks like

Dermatologists at the American Academy of Dermatology say patients describe less fullness in the face, sagging skin, wrinkles and a crepey texture [2]. A facial plastic surgery review describes the same picture: facial volume and fat are depleted, leaving wrinkles and sagging [1].

In a 2023 survey by the American Academy of Facial Plastic and Reconstructive Surgery, 60 percent of its surgeons said they were seeing more patients who came in about it [8].

Is it the drug or the weight loss?

The published literature gives two answers, and they are not settled.

  • Mostly the weight loss. A 2024 dermatology commentary framed the question as whether this is a new side effect or a natural result of losing weight quickly [4]. The AAD says it is partly caused by rapid weight loss [2].
  • Possibly more than fat. The AAD adds that these medicines affect collagen and muscle as well as fat cells, which can make the face look older [3]. A 2024 review in Aesthetic Surgery Journal proposed several mechanisms, including loss of fat under and within the skin and changes in fat-making stem cells [5]. That review is about proposed mechanisms; it did not measure faces in a trial.

A 2025 systematic review of the plastic surgery literature found 23 relevant articles, which described changes that resemble advanced aging [6]. Its authors note that the underlying studies have biases [7].

How much weight is involved

Dermatologists tie the change partly to rapid weight loss [2]. In the main trials, the average loss was large:

What the labels list, and what they do not

We read the current Wegovy, Zepbound, Foundayo and Ozempic labels on DailyMed. None lists facial fat loss, facial aging or sagging skin as an adverse reaction; the only mention of the face is swelling as a sign of an allergic reaction. That silence is not proof that facial changes do not happen; it means the labels give no rate for them.

Hair loss is on the labels, and both the Wegovy and Zepbound labels tie it to weight reduction. On Wegovy 2.4 mg, 3.3% of patients reported hair loss against 1% on placebo [12]. On Zepbound it was reported in 7.1% of women and 0.5% of men [13].

What can be done about it

The medical sources describe procedures rather than products:

  • A facial plastic surgery review lists dermal fillers, skin tightening and surgery to restore volume and manage extra skin [14].
  • The AAD lists soft tissue fillers and fat transfer among the options dermatologists offer [15].

Long-term results are an open question: in the same 2023 survey, 45 percent of surgeons said it was too soon to know how patients will do over time after injections or surgery [9]. No cream or supplement is described in these sources as restoring lost facial fat.

What has been measured is the amount of weight lost, not the speed. A 2025 study looked at existing imaging of 20 patients who took GLP-1 medicines [17]. Its authors estimate an average loss of 7% of midface volume for every 10 kg of weight lost, mostly in the fat just under the skin [18]. That is a small study, and it did not test whether a slower pace changes the result.

Talk to your prescriber

The facial plastic surgery review notes that prescribers seldom bring up the face with patients [16], so you may need to raise it yourself. Do not change your dose to slow weight loss on your own; the pace of treatment is a decision to make with your prescriber. For cosmetic concerns, a board-certified dermatologist or plastic surgeon can explain what a procedure would and would not change.
