  • Aetna (Employer and commercial plans): Aetna commercial PA policy 6450-C, the “BMI 35” weight-loss variant (page dated April 10, 2026), requires for adult weight loss a baseline BMI of 35 kg/m2 or more (documentation required; for patients switching from another weight-loss drug, the BMI at the start of any drug therapy counts). aetna.com
  • Aetna (Medicare): Aetna’s Medicare page describes Medicare GLP-1 Bridge eligibility (a CMS program): Part D or Medicare Advantage drug coverage plus BMI of 35 or more, or 30 or more with a related health condition. aetna.com
  • Blue Shield of California (Employer and commercial plans): Jan. 28, 2026: depending on the large group plan, coverage for a member newly prescribed a weight loss drug may be limited to Class III obesity (BMI >= 40), or available at BMI >= 30 or BMI >= 27 with certain conditions. blueshieldca.com
  • Cigna (Employer and commercial plans): Evernorth’s EncircleRx GLP-1 program (Evernorth is The Cigna Group’s health services division; Express Scripts is its PBM) requires a BMI of 32 or more, or 27 or more with two weight-related health issues. evernorth.com
  • Cigna (Employer and commercial plans): Cigna’s Foundayo criteria also require a BMI of 30 or more before any GLP-1 was started, or a BMI of 27 or more with at least one listed weight-related condition (the list includes hypertension, type 2 diabetes, dyslipidemia and obstructive sleep apnea). static.cigna.com
  • Highmark (Employer and commercial plans): Under the policy in Highmark’s August 2024 bulletin (fully insured commercial and ACA plans), adults starting or staying on Saxenda, Wegovy or Zepbound needed a baseline BMI of 40 or more and at least two weight-related conditions. providers.highmark.com
  • Kaiser Permanente (Federal employees): Kaiser says FEHB requirements may include enrolling in a lifestyle and guided intensive behavioral program, a baseline BMI of 27 or more with a weight-related condition, and documenting intolerance of other weight-management drugs or failure to lose and keep off at least 5% of body weight after a 3-month trial of them. choose.kaiserpermanente.org

  • UnitedHealthcare (Employer and commercial plans): In UHC’s commercial weight-loss criteria (effective 9/1/2026), one requirement is a BMI of 30 or more (above the 95th percentile for children), or a BMI of 27 or more with a weight-related condition such as dyslipidemia, hypertension, type 2 diabetes or sleep apnea. uhcprovider.com
  • UnitedHealthcare (Employer and commercial plans): In the section of UHC’s criteria for North Dakota fully insured small-group and individual plans (Essential Health Benefits), Wegovy injection, Wegovy HD and Zepbound instead require a BMI of 40 or more (for children, over 120% of the 95th percentile). uhcprovider.com
