Form Health review

What it is

Form Health is a telehealth obesity-medicine program. Members get monthly video visits with a Form clinician and a registered dietitian, messaging with their team and an app [6]. Form says it accepts most major private plans and Medicare, and bills visits, labs and medicine to insurance [5].

To join, you need a BMI of 30 or more, or 27 or more with a weight-related health risk [8]. You also need a primary care doctor you have seen within the last 12 months [7].

What it prescribes

Form’s clinicians prescribe FDA-approved medicines, and its medication page lists Wegovy, Zepbound, Foundayo, Saxenda and Contrave [9]. Its help center says it will only prescribe FDA-approved medications [10]. It does not prescribe controlled substances such as phentermine [19].

What it costs

With insurance, you pay your plan’s copay or deductible for each visit, as with an in-person doctor [17]. Form says most major private plans and Medicare cover its visits, lab tests and medicines [4], and its team handles coverage reviews and prior authorization [16]. Without insurance, or if your plan does not cover Form, the self-pay plan is $299 a month for clinician and dietitian visits, messaging and the app [1]. Labs and medicine are extra [2]. The self-pay fee is HSA and FSA eligible [3].

Signing up and cancelling

You can cancel at any time [12], by filling in a cancellation form or messaging your care coordinator in the app [13]. The subscription renews automatically until you cancel [15], and after you cancel your account closes at the end of the current billing period [14]. Form says your care team will help you plan the handover to another provider [18].

Safety and regulatory record

Form prescribes FDA-approved medications only [10]. GLPLens searched FDA’s warning-letter database in September and found no letter to Form Health about GLP-1 products. Form says the program is available to people living in the U.S. by video [11], but it does not publish a list of states.

Who it suits, and who should look elsewhere

Form Health suits people with private insurance or Medicare that covers its visits, who want an obesity-medicine clinician and a dietitian every month and already have a primary care doctor.

Look elsewhere if you are paying cash for everything. At $299 a month before labs and medicine [1], its self-pay fee is higher than the monthly fee of any other program we review. It is also not an option if you have not seen a primary care doctor in the past year.
